We are excited for another year of learning and growing! It is wonderful to see our student’s happy and healthy faces. Please call, email, or send in notes with updates on your childs’ healthcare issues or health care needs during school hours. Thank you for your help to keep your children healthy and ready to learn.

Swanson Road Intermediate School Nurse
At SWIS we are required by state law to conduct the following screenings annually:
Vision - 3rd, 4th and 5th grade
Hearing - 3rd grade
Height, weight and BMI - 4th grade
Postural - 5th grade
Consent for mandatory screenings are included in the New or Returning Student Registration forms in PowerSchool. A notice will be sent out prior to screenings notifying parents/guardians when screenings will be conducted.
NEW STUDENTS: Students new to the Auburn Public School District must present proof of immunizations BEFORE they are able to register for classes AND a copy of a recent physical exam within 13 months.
CURRENT STUDENTS: Students currently enrolled in the Auburn Public School District are required to show proof of immunization OR a letter indicating a religious or medical exemption. All 4th grade students are required to provide a copy of a current physical exam.
