Physicals/Immunizations

  • The state requires all 7th grade students to have a current physical. If you have not sent it in, please do so ASAP. You can drop them off, fax them to: 508-832-8655, or email them to Kate Sparks. Although only required in 7th grade, sending routine physicals is much appreciated!
  • A physical is required for sports annually and under MIAA guidance valid for 13 months from date of exam. Please visit the Family ID link to learn more about how to upload your child’s physical.
  • Students are required to have meningococcal and Tdap vaccinations upon entering 7th grade. It is usually done between ages 11-12 so if your 5th or 6th grader has had it, please provide a copy to the nurse's office. 

Medications

  • If your student takes medication at school, please remember that all medication, even over the counter medicine, will need a doctor’s order and a parent permission form (Parent Permission Form
  • If you have a student who currently has medication at school, this includes prescribed, over the counter medication, EpiPens, Inhalers, etc. we will need you to pick your student’s medication up by the last day of school. Any medication not picked up by parents/guardians will be disposed of properly.

Health Screenings

  • 7th grade- Vision, hearing and BMI
  • All grades- Postural screening

If your child has any of the following, please keep them at home for observation and treatment:

  • Fever of 100.0 degrees or higher
  • Vomiting or diarrhea within the last 24 hours
  • Strep throat or Impetigo-Your child must be on an antibiotic x 12-24 hours prior to returning to school
  • Untreated lice
  • Thick, crusty drainage from the eyes
  • Unusual rash
  • A cough that may be disruptive to your child or their classmates during the day
